By accessing and using Clearview OBGYN Ultrasound Clinic services, you agree to the following terms:
1. Medical Disclaimer
Our clinic provides ultrasound services only. The ultrasound report must be returned to your referring physician or midwife for appropriate medical evaluation and diagnosis. Patients are strongly advised not to self-diagnose based on imaging results.
2. Medical records
Medical records are stored electronically up to three (3) years only.
3. Appointment time
Our booking system is designed to manage the queue and does not guarantee your selected appointment time. Your actual scan time depends on the procedures ahead of you.
4. Late and Cancellation
Patients are advised to arrive at least 30 mins before their booking time. Late arrivals can still be accommodated but may rejoin at the end of the queue. Cancellation must be made at least 24 hours prior to the appointment.
